Get A Library Card

Who can get a library card?

Library cards are free to any Wisconsin resident. Anyone at least five years old may apply for a library card. Anyone under age eighteen must have a parent or legal guardian sign the statement of responsibility on the application form.

How can I get a library card?

Start your application online and get a temporary digital card (good for 30 days, must come in to complete application process) OR come in to your library and get a card in person. 

In addition to the application form, please provide:

  • A photo ID (Adults 18 and over)
  • Current address.  If your driver’s license or state ID does not have your current address on it, you may bring in a current bill (phone bill, utility bill, etc.) that is in your name with your current address on it.
  • Phone number

If you have a library card from another public library in Jefferson or Waukesha Counties, you may use that card at the Jefferson Public Library.

Holds notification – You can opt to receive notification that your items are in either by phone, email, or text message.

What if I lose my card?

Replacement cards are available. Once a replacement card has been issued old cards will not be considered valid. You must notify the library if your card is lost.

You are responsible for all items checked out on your library card. You are responsible for notifying the library of any changes to address or telephone information.
